We require all participants to sign/send in the registration and waiver before they can participate in any Game Ready Sports Association (GRSA)’s authorized activity.

All new and returning participants must complete this waiver form at the beginning of each season.

Liability Relsease - In consideration of my child’s participation in GRSA's programs, I recognize, acknowledge, and accept the inherent risks associated with physical, sports, and recreational activities. I hereby agree to release Game Ready Sports Association from all claims, liabilities, obligations, and costs which I may have against Game Ready Sports Association and their respective officers, directors, agents, employees, independent contractors, volunteers, servants and representatives, arising out of injury, loss or damage while I or my child participate in the program, whether or not arising from any negligence on the part of Game Ready Sports Association Staff or their respective officers, directors, agents, employees, independent contractors, volunteers, servants and representatives. *
